Where it comes from
A bulk edit applied to the wrong selection, an import whose category column shifted, a product duplicated from one in another department. The assignment is valid, so nothing complains.

How CatalogLift finds it
For each published product with one final category, a decision model is asked two things in one request: does this product belong in the category it is filed in, and which of the store's categories fits it. A finding opens only when the model doubts the current category and still picks a different existing one. A missing category in your tree is not a misfiled product, so "none of these" never opens a finding.
The verdict is stored against the exact product text and category list that were judged, so an unchanged product is never asked about twice.
Move it, or say it is intended
The finding shows both categories. Pick the right category and apply it after review, or dismiss the finding with a reason if the product is filed where you want it.
